Job Overview:
We are currently recruiting a Band 6 Occupational Therapist to join the community team at Manchester University NHS Foundation Trust. This is an excellent opportunity for a motivated and experienced therapist to work with a diverse caseload in a community setting, promoting independence and improving the quality of life for patients in their own homes.
The successful candidate will play a vital role in assessing and treating patients with complex needs, collaborating with multidisciplinary teams, and ensuring seamless care across the community.
Key Responsibilities:

Carry out holistic and person-centred occupational therapy assessments in patients' homes or community settings

Develop and implement rehabilitation plans to promote independence and improve functional ability

Collaborate with other healthcare professionals including physiotherapists, nurses, and social workers

Provide advice and guidance on adaptations, equipment, and assistive technology

Maintain accurate and timely clinical records in line with NHS documentation standards

Use EMIS (essential) and HIVE (desirable) systems for patient management and reporting

Support timely discharge planning and prevent hospital readmissions
Requirements:

Degree or Diploma in Occupational Therapy
-
Registration with the Health and Care Professions Council (HCPC)

Prior experience working at Band 5 level within the NHS

Proficiency in using EMIS (essential); HIVE experience is desirable

Strong clinical reasoning, communication, and organisational skills

Ability to work independently and as part of a multidisciplinary team

Full UK driving licence is preferable (community travel may be required)
