Join our Single Point of Access (SPA) Team at North Lincolnshire Council!We're looking for a dedicated Adult Social Worker to support individuals at Scunthorpe General Hospital and in the community. This role is vital in ensuring timely, person-centred care for adults needing discharge support or community-based assessments. �?

?? Key Responsibilities

�? Work within the Hospital Integrated Discharge Team�? Conduct home visits and Care Act assessments?? Complete Mental Capacity Assessments (MCA) and Best Interests (BI) decisions as needed?? Provide timely and safe discharges from hospital to home or care settings?? Liaise with health professionals, families, and community services to plan appropriate support??? Some weekend working will be required as part of the hospital discharge rota

? What We're Looking For

?? Qualified Social Worker with Social Work England registration?? Sound knowledge of the Care Act 2014, MCA, and discharge planning?? Experience in hospital discharge, community work, or adult services?? Strong assessment, documentation, and communication skills?? Ability to work under pressure and manage competing priorities
