Join our team as a Clinical Pharmacist, supporting a GP surgery and making a direct impact on patient care and medicines optimisation. In this role, you will:
- Conduct medication reviews to improve patient treatment outcomes
- Provide prescribing support and guidance to GPs
- Manage repeat prescriptions, ensuring safety and accuracy
- Oversee medicines reconciliation for new and discharged patients
- Support long-term condition management (e.g., diabetes, hypertension)
- Answer prescription queries and collaborate with healthcare teams
- Work onsite to deliver high-quality pharmacy support
To be successful in this role, you will need:
- Prior GP experience is essential
- Independent Prescribing qualification (preferable)
- Strong expertise in medicines optimisation and patient consultations
- Proficiency in GP pharmacy systems (EMIS & SystmOne)
- Excellent communication and teamwork skills
