We are looking for a highly organized and dedicated Care Coordinator to join our St Helens branch. This is a vital role in ensuring the smooth and efficient delivery of care services, supporting both our staff and Service Users.

Your Role & Responsibilities:
  • Scheduling & Coordination Organize and manage rotas to ensure Service Users receive timely, high-quality care
  • Client Assessments Assist in conducting assessments of tasks and risks to create personalized care plans
  • Supporting Care Staff Provide ongoing guidance and support to care workers, ensuring high standards are met
  • Liaising with Stakeholders Communicate with Service Users, families, and healthcare professionals to ensure seamless care delivery
  • Problem Solving Address and resolve any scheduling issues, staff absences, or last-minute changes efficiently
  • On-Call Support Participate in the out-of-hours rota (1 in 4 weekends & 1 night per weekday) to provide emergency support
