We are looking for a highly organized and dedicated Care Coordinator to join our St Helens branch. This is a vital role in ensuring the smooth and efficient delivery of care services, supporting both our staff and Service Users.
Your Role & Responsibilities:- Scheduling & Coordination Organize and manage rotas to ensure Service Users receive timely, high-quality care
- Client Assessments Assist in conducting assessments of tasks and risks to create personalized care plans
- Supporting Care Staff Provide ongoing guidance and support to care workers, ensuring high standards are met
- Liaising with Stakeholders Communicate with Service Users, families, and healthcare professionals to ensure seamless care delivery
- Problem Solving Address and resolve any scheduling issues, staff absences, or last-minute changes efficiently
- On-Call Support Participate in the out-of-hours rota (1 in 4 weekends & 1 night per weekday) to provide emergency support

- Experience in care coordination or a similar role (preferred)
- Knowledge of care planning & scheduling software (advantageous)
- Strong organizational and problem-solving skills

- Full UK driving license & access to a vehicle at all times
- Ability to work under pressure in a fast-paced environment
