Social Worker - Adults Contact and Assessment Team

Location: Stafford, Staffordshire (Hybrid - minimum 2 days per week in the office)
Pay Rate: 28.40 per hour (DOE)
Contract: Full-time, ongoing

About the Role

We are currently recruiting for an experienced Social Worker to join the Adults Contact and Assessment Team within Staffordshire County Council.

This is a hybrid assessment role, offering flexibility to work from home while maintaining a collaborative office presence. You will complete Care Act assessments primarily over the phone or via virtual platforms, with occasional face-to-face visits when required.

The work is fast-paced but not overly complex, requiring a confident practitioner who can manage their own caseload effectively, prioritise workloads, and deliver high-quality, person-centred assessments.

Key Responsibilities

Undertake proportionate assessments in line with the Care Act 2014.

Provide information, advice, and guidance to adults and carers accessing services.

Identify eligibility for support and arrange appropriate care packages.

Work closely with unqualified staff, sharing practice knowledge and supporting professional development.

Maintain accurate and timely case records using the council's case management system.

Liaise with health professionals, voluntary organisations, and community partners to ensure effective outcomes for service users.

Requirements

Qualified Social Worker with current Social Work England registration.

Previous experience within Adult Social Care, ideally in an assessment or contact team.

Strong communication and organisational skills.

Ability to work autonomously and manage a fast-paced caseload.

Confident using IT systems and virtual communication tools.
