Halcyon is proud to partner with a local service dedicated to caring for older individuals in the Tameside area. We are currently seeking an enthusiastic Extra Care Coordinator to support management in delivering high-quality Extra Care services.

About the Role:

As an Extra Care Coordinator, you will play a vital role in providing integrated, holistic care and support to older individuals with diverse needs, including dementia, mental health challenges, physical disabilities, learning disabilities, and complex health conditions.

The scheme features a variety of self-contained one and two-bedroom flats, accommodating residents who may require additional care and support within their homes.

Key Responsibilities:

  • Provide direct support to residents, which includes personal care, medication management, assistance with mobility aids, and support with daily living activities such as shopping and laundry.
  • Encourage and facilitate residents' access to community resources such as work, education, and social activities.
  • Embrace all aspects of the role, including supporting individuals through personal care and domestic tasks while incorporating your personal skills and interests (e.g., sports, arts, music, and cooking) into resident engagement.
  • Work collaboratively with the management team to ensure the smooth operation of the Extra Care services and maintain a supportive environment for both staff and residents.
