What will your responsibilities be?
In this role, you will manage casework for hospitalized adults, ensuring smooth care coordination and discharge planning. You'll assess needs, develop care plans, and facilitate transitions, while maintaining accurate records and compliance with standards. Essential Requirements:
- Social Work England registration
- Eligibility to work in the UK
- Full UK driving license
- Post-qualified experience
- Degree level or equivalent in Social Work
