Job Overview:

We are looking for an experienced Band 6 Occupational Therapist to join the community team supporting patients post-discharge from hospital. This role plays a key part in ensuring patients are assessed within 72 hours of discharge and provided with timely interventions.

Key Responsibilities:
  • Carry out assessments in patient homes within 72 hours of discharge

  • Liaise with external agencies and arrange for equipment provisions

  • Record notes on a laptop during or immediately after patient visits

  • Work independently and manage your daily caseload efficiently

  • Be based on-site at 8am and either finish at base or leave final patient by 4pm

Requirements:
  • HCPC registered Occupational Therapist

  • Experience in community or discharge-to-assess roles

  • Strong communication and time management skills

  • Must be a car driver with access to a vehicle (essential)

  • Ability to work autonomously and as part of a wider MDT
