Job OverviewWe are looking for an experienced Band 6 Occupational Therapist to join the community team supporting patients post-discharge from hospital. This role plays a key part in ensuring patients are assessed within 72 hours of discharge and provided with timely interventions.
Key Responsibilities-
Carry out assessments in patient homes within 72 hours of discharge

Liaise with external agencies and arrange for equipment provisions

Record notes on a laptop during or immediately after patient visits

Work independently and manage your daily caseload efficiently

Be based on-site at 8am and either finish at base or leave final patient by 4pm

HCPC registered Occupational Therapist

Experience in community or discharge-to-assess roles

Strong communication and time management skills

Must be a car driver with access to a vehicle (essential)

Ability to work autonomously and as part of a wider MDT
