Key Responsibilities include:

  • Respond to calls in the community, providing emergency and wellbeing support to vulnerable individuals

  • Carry out welfare checks and build rapport with service users to promote safety and independence

  • Use specialist equipment to assist and lift individuals safely if required (full training provided)

  • Provide compassionate and empathetic care at all times, ensuring dignity and respect for service users

  • Record and report all interventions accurately, maintaining clear communication with the wider team

  • Work collaboratively with colleagues—no lone working involved

Essential Criteria:

  • Enhanced DBS issued within the last 3 years

  • Full UK driving licence and access to a vehicle (essential)

  • Flexible approach to working varying shift patterns

  • Positive attitude and ability to provide calm, reassuring support in challenging situations

  • Previous experience in care coordination or housing support
