An experienced and motivated Occupational Therapist is required for a short-term assignment within a community-based adult social care team. The role focuses on promoting independence, reducing care needs, and supporting safe discharge from hospital through tailored Occupational Therapy interventions.
Key Responsibilities:-
Deliver comprehensive OT assessments to identify needs and recommend appropriate interventions.

Promote independence by prescribing equipment, assistive technology, and manual handling solutions.

Provide tailored training to service users and their carers on safe use of equipment and techniques.

Review post-discharge care packages to ensure appropriate support is in place while minimising dependency.

Collaborate with care providers, health professionals, and other agencies to deliver joined-up support.

Maintain accurate case notes and contribute to multi-disciplinary case planning.

HCPC-registered Occupational Therapist.

Previous experience working in adult social care or community rehabilitation.

Proficient in assessing for and prescribing equipment and adaptations.

Strong understanding of post-discharge pathways and early intervention approaches.

Able to work independently, manage a caseload, and make informed clinical decisions.
