- Follow care and support plans for each service user and tenant with respect and dignity.
- Promote living independently by maintaining good health, hygiene and personal care e.g. dressing, washing, shaving, going to the toilet.
- Accessing work, education or training.
- Enjoying leisure time, activities and cultural beliefs in the community.
- Keeping in touch with family and friends and making new friends.
- Carry out or assist with domestic cleaning, cooking and shopping tasks as instructed.
- Assist with the provision of medication as directed.
- Provide up to date information to maintain service user records as required.
- Assist in delivery of clients who may present behavioural challenges.
- Use IT systems to record information and correspond with team and stakeholders.
- Provide coaching to other team members which includes observing practise and giving feedback.

- Understand and comply with HCHAs policies and procedures on Finance, Technology Services, Data Protection, Health and Safety, Equality and Diversity and any other departmental policies in relations to the key responsibilities.
- Experience supporting clients with behaviours that challenge, none verbal, autistic, require personal care.
- Must hold UK driving License
- Enhanced DBS with barred list check (desirable)
