- Work collaboratively with NHS and community teams to support early discharge planning.
- Assess needs and coordinate timely support packages for customers ready to return home.
- Ensure decisions are person-centred and promote independence.
- Maintain accurate and timely documentation in accordance with policies and procedures.
- Qualified Social Worker with current Social Work England registration.
- Strong experience in hospital discharge or adult social care settings.
- Ability to work effectively within a multi-disciplinary team.
- Confident decision-making and problem-solving skills under pressure.
- Knowledge of discharge to assess (D2A) models and relevant legislation (e.g., Care Act 2014).
- Experience in fast-paced hospital or health settings.
