Job Title: Band 6 Occupational Therapist - Discharge to Assess Ref: GDJ62 Location: Musgrove Park Hospital - Community Services (Primary Care & Neighbourhoods) Service: Discharge to Assess (D2A) Grade: Band 6 Contract Type: Full Time preferred (Part Time considered) Start Date: 06 May 2025 End Date: 19 September 2025 Hours: Monday to Friday, 09:00 - 17:00 Rate: Agenda for Change - Band 6 £25.48 umbrella equivalent Role Overview: We are seeking an experienced Occupational Therapist (Band 6) to join the Discharge to Assess (D2A) team within the community services arm of Somerset NHS Foundation Trust. Based from Musgrove Park Hospital, the role is community-focused and delivers essential reablement support to patients following hospital discharge, enabling them to regain independence in their own homes. The successful candidate will provide short-term interventions and assessments within the D2A framework, working closely with reablement providers, patients, families, and wider health and social care professionals. Key Responsibilities: Undertake holistic assessments of patients in their own homes post-discharge Deliver reablement-focused occupational therapy interventions Liaise with D2A trained care provider agencies to ensure seamless patient support Contribute to goal setting and rehabilitation planning tailored to individual needs Promote independence and reduce the need for long-term care packages Work across geographical clusters and be flexible to support county-wide if needed Maintain accurate clinical records and participate in service development activities Essential Requirements: HCPC registered Occupational Therapist Recent community reablement experience working with adults Car driver with access to own vehicle (role involves home visits) Strong assessment and discharge planning skills Ability to work autonomously and as part of a multidisciplinary team Flexible to travel across Somerset to meet service needs Desirable: Previous experience within Discharge to Assess services Knowledge of local community support structures and D2A care pathways Additional Notes: This is a fantastic opportunity to contribute to an essential community-focused service that supports people to safely transition from hospital back into their homes.
