Tripod Partners are looking for two Band 6 Urgent Community Response Practitioners to join a dynamic community health team in Teddington. This is a cross-disciplinary role, with a preference for nurses, and focuses on rapid response interventions to prevent unnecessary hospital admissions.

Key Responsibilities:

  • Deliver urgent care interventions in patients’ homes or community settings.

  • Work collaboratively across disciplines to assess, plan, and implement patient-centred care.

  • Support rapid response to referrals, often within two hours.

  • Help manage acutely unwell patients in the community to avoid hospital admission.

  • Coordinate with wider health and social care teams for holistic support.

Requirements:

  • NMC registration (for nurses) or appropriate professional registration for allied roles.

  • Experience in community or urgent care settings.

  • Ability to assess complex needs and act quickly in time-critical situations.

  • Strong communication and teamwork skills.
