Job Title: Band 6 Urgent Community Response Practitioner
Location: Teddington
Pay Rate: £29.02 per hour
Duration: 3 months
Work Pattern: Monday to Friday

Tripod Partners are looking for two Band 6 Urgent Community Response Practitioners to join a dynamic community health team in Teddington. This is a cross-disciplinary role, with a preference for nurses, and focuses on rapid response interventions to prevent unnecessary hospital admissions.
Key Responsibilities:

Deliver urgent care interventions in patients’ homes or community settings.

Work collaboratively across disciplines to assess, plan, and implement patient-centred care.

Support rapid response to referrals, often within two hours.

Help manage acutely unwell patients in the community to avoid hospital admission.

Coordinate with wider health and social care teams for holistic support.
Requirements:

NMC registration (for nurses) or appropriate professional registration for allied roles.

Experience in community or urgent care settings.

Ability to assess complex needs and act quickly in time-critical situations.

Strong communication and teamwork skills.

Right to work in the UK.
