Tripod Partners are seeking a Band 7 Urgent Community Response Practitioner to join a high-performing community team in Teddington. This is a cross-discipline role, with a preference for nurses, supporting the urgent care needs of patients in the community to avoid unnecessary hospital admissions.

Key Responsibilities:

  • Provide advanced clinical assessments and rapid interventions for acutely unwell patients in community settings.

  • Lead urgent response efforts within 2-hour timeframes.

  • Coordinate multi-disciplinary care plans, ensuring safe and effective treatment at home.

  • Support and supervise junior team members across disciplines.

  • Collaborate with primary care, acute trusts, and social services to deliver integrated care.

Requirements:

  • NMC registration (for nurses) or other appropriate professional registration.

  • Proven experience in urgent or community care at Band 6 or Band 7 level.

  • Clinical leadership and decision-making skills in high-pressure situations.

  • Ability to work autonomously and across multi-agency teams.

  • Right to work in the UK.
