- Work directly with young people to promote their independence, resilience, and positive future outcomes.
- Support care leavers in transitioning into adulthood, including housing, education, employment, and wellbeing.
- Undertake assessments, develop care/pathway plans, and review progress in line with statutory requirements.
- Work collaboratively with multi-agency partners to deliver coordinated support.
- Ensure young people are actively involved in their own plans and contribute to service development.
- Recognised social work qualification (Degree, DipSW, CQSW, or equivalent).
- Registration with Social Care Wales.
- Experience of working with children, young people, and their families/carers in statutory settings.
- Experience of multi-agency working.
- Knowledge of relevant legislation including the Children Act and the Social Services and Well-being (Wales) Act.
