Halton Borough Council is seeking a dynamic and experienced Principal Manager to oversee the Adult Service Hospital Discharge and Urgent Care social care team. This critical role will lead, develop, and manage service delivery to ensure the best outcomes for adults transitioning from hospital to home and those requiring urgent social care support.

Key Responsibilities:

  • Provide strong leadership and operational management to the Hospital Discharge and Urgent Care social care team.
  • Develop and implement strategic plans to improve service delivery, ensuring compliance with statutory guidelines and local policies.
  • Collaborate effectively with health partners, external agencies, and internal teams to drive seamless care transitions.
  • Oversee assessments, care planning, and risk management for complex cases.
  • Ensure the delivery of person-centred, strength-based approaches to social care.
  • Monitor and evaluate service performance, implementing improvement strategies where necessary.
  • Manage budgets effectively, ensuring resource allocation aligns with service priorities.

Requirements:

  • Qualified Social Worker with current registration with Social Work England.
  • Substantial experience in adult social care, particularly in hospital discharge and urgent care settings.
  • Proven leadership experience, with a track record of managing social care teams effectively.
  • Excellent knowledge of relevant legislation, policies, and best practices in adult social care.
  • Strong communication, negotiation, and decision-making skills.
