Health Care Coordinator

We are seeking a dedicated Health Care Coordinator (Registered Nurse) to support our Client’s team and the individuals they support, who are adults with diverse learning abilities. This role is crucial in creating and implementing effective health care plans, promoting health, and providing training on health-related issues.
Day-to-Day Responsibilities:
- Develop and implement effective health care plans in collaboration with the individuals we support and their care teams.
- Act as an advocate for individuals, helping them access necessary health care services and challenging health inequalities.
- Provide training and education on health-related issues to both the individuals we support and their care teams.
- Supervise health-related practices in our houses, ensuring all care is person-centred and meets individual needs.
- Create health-related passports for individuals to assist in areas such as meal support and incontinence support.
- Maintain effective communication with individuals we support, their care teams, and management.
- Work closely with the Healthy Living Team Leader and other health professionals to ensure a holistic approach to health.
Required Skills & Qualifications:
- Minimum of five years’ experience in health care or nursing.
- Relevant professional qualification in nursing (Note: this position does not meet the criteria for continued RGN status).
- Proven IT skills to create Easy Read materials and health-related documents.
- Experience working with individuals with diverse learning abilities.
- Excellent interpersonal and communication skills, with the ability to motivate and educate others.
- Ability to plan, prioritise, and manage workload effectively in a demanding environment.
- Calm and reflective approach in crisis situations.
- Flexibility and teamwork skills, with a commitment to delivering a quality, person-centred service.
Special Conditions:
- DBS check required.
- Driving license required.
- Must complete relevant Induction.
This role occasionally requires work outside normal hours for specific functions, and supervision sessions are held every 8 weeks with follow-ups by registered managers. This position is particularly suited for those seeking a less demanding role in their later professional years or those looking to focus more on training and staff development and heath care plans.
