Job Description:
I am looking for a dedicated and compassionate Senior Occupational Therapist to join our multidisciplinary team. The ideal candidate will provide high-quality, patient-centred care to adults with physical, mental, or sensory impairments to enhance their daily living skills, improve independence, and promote well-being.
The role involves working with adults in various settings, such as hospitals, community health services, or care homes. You will conduct assessments, develop personalised treatment plans, and deliver therapeutic interventions to individuals facing challenges in their daily activities due to illness, injury, or disability.
Key Responsibilities:
- Assessment: Conduct comprehensive assessments of individuals' physical, cognitive, and emotional needs to identify barriers to daily activities and determine appropriate interventions.
- Care Planning: Develop and implement personalised care plans to improve clients' functional abilities, independence, and quality of life. Collaborate with healthcare teams to ensure holistic and coordinated care.
- Therapeutic Intervention: Deliver evidence-based interventions to support clients in achieving their goals. This may include teaching techniques for tasks like dressing, cooking, bathing, or working, and recommending adaptive equipment.
- Rehabilitation: Work with individuals recovering from illness, surgery, or injury to restore function, mobility, and independence. Support clients with neurological conditions, stroke rehabilitation, or orthopaedic recovery.
- Education: Provide education and training to clients and their families on managing daily activities, using assistive technologies, and maintaining health and well-being at home.
- Care Coordination: Liaise with other healthcare professionals (doctors, physiotherapists, social workers, etc.) to ensure a multidisciplinary approach to patient care. Maintain accurate and up-to-date case notes and treatment records.
Requirements:
- Professional qualification in Occupational Therapy and current registration with Health and Care Professions Council (HCPC)
- Experience of working within a social care, health or community focused multi-disciplinary setting
- Ability to complete person centred and proportionate OT assessments
- Knowledge of medical conditions and the likely impact on the individual, carer and family
- Knowledge of current legislation, including the Care Act and the Mental Capacity Act and its application to practice in a social care setting.
