Job Title: Social Worker - D2A/Safeguarding TeamLocation: WolverhamptonPay Rate: £ 33.30/per hourLiquid Personnel is seeking an experienced social worker to join their Discharge to Assess (D2A) and Safeguarding Team. This role involves leading and managing safeguarding processes to protect and promote the well-being of adults at risk, children, carers, family members, and vulnerable individuals. The successful candidate will be responsible for delivering high-quality safeguarding practices and driving continuous improvement within the team.

Responsibilities:
  • Caseload Management: Manage a diverse caseload of safeguarding cases, prioritizing based on urgency and need, ensuring timely interventions and support.
  • Safeguarding: Identify and respond to safeguarding concerns, conducting investigations, and implementing protection plans to ensure the safety of vulnerable individuals.
  • D2A Function: Undertake the Discharge to Assess function, ensuring smooth transitions from hospital to home or other care settings, evaluating needs and coordinating care plans.
  • Multi-Disciplinary Collaboration: Collaborate with a range of professionals, including healthcare providers, therapists, and community organizations, to provide holistic support.
  • Strategy Meetings: Chair multi-agency Strategy Meetings, Adult Protection Case Conferences, and other professional meetings as required.
  • Crisis Intervention: Provide immediate support and intervention in crisis situations, working to stabilize and support individuals and their families.
  • Record Keeping: Maintain accurate and up-to-date records of assessments, care plans, and interventions, ensuring compliance with legal and organizational standards.
  • Care Planning: Develop, implement, and review personalized care plans in collaboration with individuals, their families, and other professionals to meet their needs.
  • Knowledge Sharing: Ensure the transfer of knowledge and sharing of best practices across the team, Adult Services, and partner agencies.

Qualifications and Experience:
  • Professional Qualification: A recognized professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England, the regulatory body for social workers in England.
  • Experience: Significant experience in delivering social work services to vulnerable adults, including those with complex needs.
  • Knowledge of Legislation: Thorough understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005.
  • Skills: Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.
  • Driving Licence: A full UK driving licence and access to a car are often required for client visits.
