- Conduct comprehensive assessments of adults with neurodevelopmental conditions, identifying their needs and developing person-centered support plans.
- Provide direct support and guidance to individuals and their families, empowering them to manage challenges and achieve their goals.
- Collaborate with multi-agency partners (health professionals, housing providers, support organizations) to coordinate care and resources.
- Advocate for the rights and needs of neurodivergent adults, promoting inclusion and understanding.
- Contribute to the development and improvement of services for neurodivergent adults within Worcestershire.
- Manage a caseload effectively, balancing competing priorities and ensuring timely interventions.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience working with adults with neurodevelopmental conditions is highly desirable.
- Excellent assessment, communication, and case management skills.
- Strong understanding of autism, ADHD, and other neurodevelopmental conditions.
- Ability to build rapport and trust with neurodivergent individuals and their families.
- Commitment to person-centered practice and ethical standards.
