This role offers the opportunity to support individuals across the lifespan, from children to adults, experiencing a wide range of disabilities.About the Role:
- Conduct thorough assessments of individuals with disabilities, identifying their needs and developing person-centered support plans.
- Provide direct support and guidance to individuals and their families, empowering them to live fulfilling lives.
- Collaborate effectively with multi-agency partners (health professionals, education providers, support organizations) to coordinate care and resources.
- Advocate for the rights and needs of individuals with disabilities, promoting inclusion and access to appropriate services.
- Manage a diverse caseload, prioritizing urgent needs and ensuring timely interventions.
- Contribute to the ongoing development and improvement of services for individuals with disabilities within Worcestershire.
Requirements:
- Registered with Social Care England.
- Minimum 3 years' post-qualification experience as a qualified Social Worker. Experience working with individuals with disabilities is highly desirable.
- Excellent assessment, communication, and case management skills.
- Strong understanding of relevant legislation and policies relating to disability social care.
- Ability to build rapport and trust with individuals with diverse needs and their families.
- Commitment to person-centered practice and ethical standards.
