What You'll Be Doing:

Join our team as an Adults CMHT CPN and play a vital role in supporting adults with complex mental health needs. Your responsibilities will include:

? Conducting comprehensive mental health assessments for adults in the community.? Developing and implementing personalised care plans in collaboration with a multidisciplinary team.? Providing evidence-based therapeutic interventions to support mental well-being and recovery.? Offering crisis intervention and risk management strategies to ensure patient safety.? Liaising with families, social workers, GPs, and other professionals to deliver holistic care.? Maintaining accurate and up-to-date patient records in line with NHS standards and guidelines.? Supporting patients in managing their conditions, promoting independence and quality of life.

Requirements & Qualifications:

To be successful in this role, you will need:

? A current and valid NMC registration (or relevant professional body registration).? Previous experience in Community Mental Health or working with adults in mental health settings.? Strong communication and interpersonal skills.? The ability to work independently and within a multidisciplinary team.? A commitment to delivering high-quality, patient-centered care.

Working Hours & Contract Duration:

This role requires availability from Monday to Friday, 9:00 AM - 5:00 PM. The contract is for an initial 3-month period with a strong possibility of extension based on performance and service needs.
