Position Overview:

We are seeking a dedicated and compassionate ChildrenandFamilyWorker to join our Childrenwith Disabilities Team. This role offers a fantastic opportunity to make a meaningful impact on the lives of childrenwith disabilities and their families. The ideal candidate will have excellent phone skills, a solid understanding of the education system, and a genuine passion for supporting childrenwith disabilities.

Key Responsibilities:

Direct work with children and families: Provide direct support and guidance to children with disabilities and their families, addressing their individual needs and challenges. This may include practical support, emotional support, advocacy, and signposting to other services.

Assessment and Care Planning: Conduct thorough assessments of children's needs and develop person-centred care plans in collaboration with families and other professionals.

Multi-Agency Working: Work effectively with a range of professionals, including health professionals, educational professionals, therapists, and other support services, to ensure holistic and coordinated support for children and families.

Advocacy: Advocate for children's rights and needs, ensuring they have access to appropriate services and resources.

Case Management: Manage a caseload of children and families, ensuring timely and effective service delivery.

Record Keeping: Maintain accurate and comprehensive records of all contacts and interventions.

Training and Development: Participate in relevant training and development opportunities to enhance professional skills and knowledge.

Essential Criteria:

  • Relevant professional qualification (e.g., social work, youth work, or equivalent).
  • Experience working with children with disabilities and their families.
  • Understanding of the needs of children and families affected by disability.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to build rapport and trust with children and families.
  • Ability to work independently and as part of a team.
  • Full UK driving license and access to a vehicle (may be required).
