Experienced Social Worker - Adults Single Point Of Access
Up to £40 per hour
We are seeking a highly motivated and skilled Experienced Social Worker to join our dynamic Adults Single Point of Access (SPOA) team. This is a crucial role within our service, acting as the front door to Adult Social Care and working collaboratively with our Early Response team.
About the Role:
As an integral member of the Adults SPOA, you will be responsible for:
- Screening and responding to all new referrals into Adult Social Care with efficiency and professionalism.
- Undertaking initial assessments to understand individual needs and circumstances.
- Determining appropriate next steps in line with legislation, policy, and best practice.
- Providing comprehensive Information, Advice, and Assistance (IAA) to individuals, carers, and families.
- Effectively signposting to relevant internal and external services and support networks.
- Coordinating the offer of IAA to ensure timely and appropriate support is provided.
- Working in a collaborative and person-centred way, ensuring the voice and choice of individuals are central to all interventions.
- Maintaining accurate and up-to-date records in accordance with relevant procedures.
- Contributing to the ongoing development and improvement of the SPOA service.
About You:
You will be a qualified and registered Social Workerwith significant post-qualification experience working with adults in a social care setting. You will embody our core values of:

Hybrid Working:
This is a hybrid role based at our Crown Buildings in Wrexham. You will be required to have a presence in the office for 3 days per week, fostering team collaboration and access to resources. For the remainder of the week, you will have the flexibility to work from home, supported by all the necessary technology and equipment.
Important Note: As part of our commitment to providing a responsive service, successful applicants must be able to respond from their home working location should an emergency arise that necessitates office presence.
