RN or LPN – Care Center Nurse

Location: Afton, WY
Facility: Star Valley Health Care Center
Job Status: Full-Time

Job Type: 3 Full-time Night Shifts
License Required: RN or LPN (Wyoming)

New Employee Bonus: Available for full & part-time employees
Salary Range:

  • RN: $33.65–$50.49/hour (depending on experience)
  • LPN: $22.68-$34.03/hour (depending on experience)

Key Responsibilities:

  • Review CNA assignments and communicate ongoing resident needs throughout the shift.
  • Supervise, direct, and motivate CNAs to ensure quality care aligned with each resident’s plan of care.
  • Assess residents through observation, conversation, and physical evaluation; document findings accurately.
  • Provide direct supervision and coordination of the resident care team.
  • Administer medications, assist with ADLs, treatments, and therapies within scope of practice.
  • Report changes in resident condition to physicians and families; document updates in medical records.
  • Arrange emergency or outpatient services as needed.
  • Follow facility policies, procedures, physician orders, and state/federal guidelines.

Requirements

Qualifications:

For RNs:

  • Graduate of a state-approved nursing program.
  • Current RN license in the State of Wyoming.
  • Completion of SV Health/SVCC orientation program.

For LPNs:

  • Graduate of a state-approved nursing program.
  • Current LPN license in the State of Wyoming.
  • Completion of SV Health/SVCC orientation program.

  • Sound judgment and problem-solving abilities.
  • Leadership and interpersonal relationship skills.
  • Proficient in resident assessment and care planning.
