REGIONAL MEDICAL SOCIAL WORKER

(Ahoskie/Swan Quarter)

Job Description:

  • Provides patient care services in regional group of Liberty Home Health & Hospice offices and documents patient care data.
  • Provides initial psycho-social assessment for the development of an individualized care plan and establishes follow-up treatment goals.
  • Applies a working knowledge of psycho-social principles and family dynamic development.
  • Identifies and reassesses social, emotional, financial, and environmental factors which may affect the medical plan of treatment and desired outcome.
  • Communicates as necessary with intercompany and intra-company contacts.
  • Utilizes community resource planning and community organization skills in providing patient services.
  • Educates patients/families on how to utilize community resources available and provides discharge instructions.
  • Other job duties as assigned.
Job Requirements:
  • Master’s degree from a school of social work that is accredited by the Council on Social Work Education and eligible for certification by the North Carolina Board of Social Work.
  • Baccalaureate degree in social work, psychology, sociology, or other field related to social work, and has at least one (1) year of social work experience in a healthcare setting
  • Minimum of one year practical experience, preferably in the home health care environment.
  • Strong interpersonal skills, with the ability to effectively communicate within a multidisciplinary setting.
  • Must accept travel and working conditions as assigned.
