What You'll Do

Every day is different.

You'll serve as an advocate, counselor, problem solver, and trusted resource for older adults and their caregivers.

Responsibilities include:

• Conduct psychosocial assessments and ongoing participant evaluations
• Develop individualized care plans with the interdisciplinary team
• Complete home visits to assess safety, support systems, and barriers to independence
• Provide counseling, crisis intervention, and emotional support
• Connect participants and families with community resources
• Assist with advance care planning and care transitions
• Participate in interdisciplinary team meetings
• Support participants experiencing housing, caregiver, financial, behavioral health, or social challenges
• Coordinate with hospitals, nursing facilities, home care agencies, and community partners
• Ensure documentation meets CMS and organizational standards

Qualifications

Required:

• Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) in Ohio
• Master's Degree in Social Work
• Valid driver's license and reliable transportation
• Ability to travel throughout the service area for participant visits as needed
• Excellent communication and documentation skills

Preferred:

• Experience with older adults, geriatrics, Medicare/Medicaid, hospice, home care, behavioral health, or case management
• Knowledge of community resources and long-term care services
• Experience working in an interdisciplinary healthcare setting

The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive medical and social services to adults age 55+ who qualify for nursing home-level care but wish to continue living safely in their own homes.

Through one integrated team—including physicians, nurses, therapists, social workers, home care, transportation, nutrition, and recreation—we care for the whole person.
