What You'll Do

Every day is different.

You'll serve as an advocate, counselor, problem solver, and trusted resource for older adults and their caregivers.

Responsibilities include:

  • Conduct psychosocial assessments and ongoing participant evaluations
  • Develop individualized care plans with the interdisciplinary team
  • Complete home visits to assess safety, support systems, and barriers to independence
  • Provide counseling, crisis intervention, and emotional support
  • Connect participants and families with community resources
  • Assist with advance care planning and care transitions
  • Participate in interdisciplinary team meetings
  • Support participants experiencing housing, caregiver, financial, behavioral health, or social challenges
  • Coordinate with hospitals, nursing facilities, home care agencies, and community partners
  • Ensure documentation meets CMS and organizational standards

Qualifications

Required:

  • Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) in Ohio
  • Master's Degree in Social Work
  • Valid driver's license and reliable transportation
  • Ability to travel throughout the service area for participant visits as needed
  • Excellent communication and documentation skills

Preferred:

  • Experience with older adults, geriatrics, Medicare/Medicaid, hospice, home care, behavioral health, or case management
  • Knowledge of community resources and long-term care services
  • Experience working in an interdisciplinary healthcare setting
