Summary

SUMMARY: The AHD Licensed Vocational Nurse assumes responsibility and accountability for a group of residents for a designated time frame and provides care to these residents via the nursing process, in collaboration with health care team members, under the direction of the Registered Nurse.  He/she is also responsible for the direction and supervision of assigned personnel.

DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.

1.  Performs data collection in an ongoing and systematic manner, focusing on physiologic, psychological, spiritual, and cognitive status and reports to R.N.

2.  Contributes to and updates a plan of care which is prioritized and based on determined nursing diagnosis and resident outcomes. Establishes priorities of resident care based on essential patient needs. Implements care in a knowledgeable, skillful, consistent, and continuous manner.

3.  Documents effectiveness of plan of care in progressing resident toward positive outcomes.

4.  Collaborates with Registered Nurse to identify resident/Primary Support Person (PSP) needs and participates in teaching resident/PSP education.  Understands and adheres to hospital policies regarding resident/PSP rights.

5.  Demonstrates adherence to safety and legal requirements established at the hospital and department level.

6.  Demonstrates awareness and has working knowledge of unit structure standards, nursing policies and protocols and administrative policies.

7.  Adheres to and supports the LVN role as defined by the LVN scope of practice and professional practice as defined by organization.

8.  Recognizes own limitations and education needs, improving knowledge and skill by participating in in-service and continuing education classes.

9.  Participates in unit goal formation and assists in projects to achieve goals.  Participates in QA/PI activities at the unit level.

10.  Communicates effectively with co-workers, physicians, residents and residents’ PSP.  Formulates and maintains effective working relationships.

11.  Demonstrates ability to organize workload and set priorities.

12.  Identifies problems, seeks out relevant data and demonstrates problem solving skills.

MINIMUM QUALIFICATIONS: Education: Graduate of an accredited vocational school of nursing. Minimum Experience: One year recent experience as a LVN in a Long Term Care or Sub-Acute setting. Preferred Experience: Experience with tracheostomy and ventilator care. Required Licenses/Certifications: Current licensures in the State of California to practice vocational nursing. Required Licenses/Certification: Basic Life Support (BLS) certification issued by American Heart Association. Required Licenses/Certification: IV Certification from Board of Vocational Nursing and Psychiatric Technicians.

 

PAY RANGE: $50.89 -  $54.83/HR

The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate’s experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization.
