What You'll Do
- Evaluate patients and create individualized treatment plans
- Deliver skilled physical therapy services in the home setting
- Partner with physicians, nurses, and interdisciplinary team members
- Educate patients and caregivers on treatment goals and home exercise programs
- Complete timely and accurate clinical documentation
What You'll Bring
- Current Oregon Physical Therapist License
- Valid driver's license and reliable transportation
- Home health experience preferred, but not required
- Strong communication and clinical skills
- A passion for helping patients achieve their goals
