Position Details
Location: Vista House Women and Children's Reintegration Residence
111 McCarty Ave, Albany, NY 12202
Schedule: Monday–Friday, 9:00 AM–5:00 PM
Flexible scheduling may occasionally be required to maintain appropriate staffing.
What You'll Do
As a Residential Addictions Case Manager, you will:
- Provide substance use counseling and case management services to women at the reintegration level of care.
- Maintain a caseload of up to 15 residents, coordinating and supporting individualized treatment goals.
- Develop, coordinate, and implement individualized treatment plans.
- Complete an Initial Service Plan within 72 hours of admission.
- Prepare and maintain accurate case records documenting resident goals, objectives, progress, and expected completion timeframes.
- Provide consistent motivation, encouragement, and emotional support as residents work toward their recovery goals.
- Monitor residents for medical, mental health, safety, and security concerns and communicate issues appropriately.
- Collaborate closely with the Program Manager and treatment team to ensure each resident's needs are being addressed.
- Monitor medication schedules while following all program policies, procedures, and protocols.
- Orient new residents to the rules, expectations, and regulations of the residence and/or supportive living program.
- Conduct and document urine drug screens and breathalyzer testing.
- Participate in weekly Case Conferences, staff meetings, resident meetings, and team meetings.
- Maintain a safe, respectful, structured, and recovery-focused residential environment.
- Perform other duties as assigned by the Program Manager.
What We're Looking For
Required Credentials & Qualifications
- CASAC-T, CASAC-P, or other CASAC credential required.
- Valid New York State Driver's License with a clean driving record.
- Minimum one year of counseling experience in a substance use disorder facility.
- Experience in a residential program serving adults with substance use disorders is strongly preferred.
Education
- Associate's Degree or higher in Psychology, Social Work, Chemical Dependency Counseling, or a related field preferred/required.
- Relevant work experience may be considered in place of a degree.
- High School Diploma or GED required.
