As a Licensed Practice Nurse (LPN), you will make a meaningful impact by delivering skilled and compassionate, one-on-one, patient centered care directly in patients' homes. Compensation

Compensation is based on experience, shift type, and market factors. The listed range reflects entry-level to highly experienced applicants.

Key Responsibilities

• Provide skilled, in-home nursing care to assigned patients
• Develop and follow individualized care plans
• Accurately document patient care, observations, and updates
• Collaborate with interdisciplinary care teams
• Maintain compliance with DOL, CMS, and state regulatory requirements

Qualifications

• Current LPN license in the state of [NM]
• Current CPR certification
• Valid driver's license and reliable, insured vehicle
• Home health experience preferred but not required
• Must pass pre-employment screenings
