Diablo Valley Post Acute is seeking an organized, personable, and detail-oriented Admissions Coordinator to join our team. This is an exciting opportunity for someone who thrives in a fast-paced healthcare environment and enjoys helping patients and families during the admission process.

As the Admissions Coordinator, you'll serve as one of the first points of contact for incoming residents, referral sources, and families, helping ensure a smooth transition into our facility while supporting census growth and delivering exceptional customer service.

Position Summary

The Admissions Coordinator is responsible for coordinating resident admissions, communicating with hospitals and referral partners, verifying admission information, and working closely with the interdisciplinary team to ensure a seamless admission experience. The ideal candidate is highly organized, customer-focused, and able to manage multiple priorities with professionalism and compassion.

Essential Responsibilities

  • Coordinate all aspects of the admissions process from referral through resident admission.

  • Communicate with hospitals, case managers, discharge planners, physicians, and referral partners.

  • Welcome residents and families, providing exceptional customer service throughout the admission process.

  • Verify insurance coverage and collect required admission documentation.

  • Collaborate with nursing, therapy, social services, and administrative teams to facilitate timely admissions.

  • Maintain accurate admission records and census reports.

  • Respond promptly to inquiries regarding admissions and facility services.

  • Assist with facility tours for prospective residents and families.

  • Build and maintain positive relationships with referral sources and healthcare partners.

  • Ensure compliance with facility policies and applicable federal and state regulations.

Qualifications

  • Previous admissions, healthcare, customer service, or administrative experience preferred.

  • Experience in a skilled nursing facility, rehabilitation center, hospital, or post-acute setting is highly desirable.

  • Strong communication and interpersonal skills.

  • Excellent organizational skills with the ability to multitask in a fast-paced environment.

  • Proficiency with Microsoft Office and electronic medical record systems is a plus.
