Job Title: Team Leader Clinical Service-Home Care
Department: Rochester Regional Health-Home Care
Location: 330 Monroe Ave, Rochester, NY 14607
Hours Per Week: 40 hours per week
Schedule: Days, 8-hour shifts, Monday - Friday, 8:30 a.m. - 5:00 p.m.
 

SUMMARY:

This position leads and/or provides oversight to a team of clinicians and support staff in providing direct clinical services to Home Care patients. This role may include assisting in the orientation of new staff, role modeling, co-visiting to evaluate or demonstrate clinical skills, participating in quality assurance activities, identifying in-service needs, designing in-service programs to address identified needs, and supervising staff.

RESPONSIBILITIES:

  • Staff Leadership & Development: Leads orientation and training for new staff, provides clinical guidance, supports ongoing development, and monitors individual performance.

  • Clinical Oversight: Ensures timely and accurate referrals, guides case management, oversees service delivery, and maintains documentation standards.

  • Program & Education Support: Participates in developing in-service programs, special clinical initiatives, and staff education sessions.

  • Compliance & Representation: Adheres to agency standards, privacy regulations, and corporate ethics; represents the agency in community meetings.

  • Operational Accountability: Conducts staff meetings, ensures attendance reliability, and performs additional functions as assigned by management.

REQUIRED QUALIFICATIONS: 

  • Bachelors’ degree that aligns with license

  • One year of experience working in a professional Home Care setting

  • Valid NYS license in Social Work, Physical Therapy, Occupational Therapy or Speech Therapy

 

PREFERRED QUALIFICATIONS:

  • Knowledge of current community health nursing practices.

  • Strong leadership/supervisory skills.

  • Solid interpersonal, organizational and time management skills.

  • With training provided, can demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.

  • Must have ability to travel.

PHYSICAL REQUIREMENTS:

Sedentary - Sedentary roles are primarily office-based and require prolonged sitting (67–100% of the workday) with minimal physical effort and lifting limited to under 10 lbs. Fine motor skills are necessary for computer work, writing, and telephone use, supported by clear visual and auditory ability. Cognitive and emotional demands are high due to extended concentration, frequent interruptions, independent decision-making, and information processing. Although physical exposure is low, administrative areas within healthcare environments may occasionally involve contact with infectious agents. Some roles may require flexible hours, TB screening, or a valid driver’s license.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
