Join UVM Health’s Adult Home Care team and help patients maintain independence, improve mobility, and enhance overall well-being—all in the comfort of their own homes. We are looking for therapists that are available weekdays, no weekends available at this tie. Must have prior PT experience; home health experience preferred but not required!

As a home health physical therapist, you’ll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff , providing comprehensive, patient-centered care.

About the Role

As an Adult Home Care Physical Therapist, you will:

  • Perform comprehensive evaluations and assessments

  • Develop individualized treatment plans tailored to each patient’s needs ✍️

  • Deliver skilled therapy interventions to improve mobility, strength, and function

  • Support patients in remaining in their homes safely and independently

Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties. Per diem support all of the 4 territories.

Minimum Qualifications âś…

  • Bachelor’s degree or higher in Physical Therapy

  • 2+ years of general PT experience, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance

  • Home health experience preferred, but not required, we can train the right person!

  • Current Vermont Physical Therapist license, or willingness to obtain one
