Arbor Post Acute is seeking a dedicated and compassionate Physical Therapist to join our skilled nursing and rehabilitation team. If you’re passionate about helping patients regain strength, mobility, and independence in a supportive, collaborative environment, we’d love to meet you.
About the Role
As a Physical Therapist, you will evaluate and treat patients following physician referrals, develop individualized treatment plans, and collaborate closely with an interdisciplinary care team. You’ll play a key role in patient recovery, staff education, and discharge planning while maintaining accurate and timely clinical documentation.
Key Responsibilities
Evaluate patients promptly and develop effective, physician-approved treatment plans
Provide hands-on therapy services and monitor patient progress
Supervise Physical Therapy Assistants, aides, and students in accordance with state practice acts
Collaborate with physicians, nursing staff, families, and the rehab team
Participate in care conferences, utilization reviews, and discharge planning
Educate patients, families, and staff on therapeutic programs and safe mobility
Maintain compliant documentation and recommend appropriate durable medical equipment
Qualifications
Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred)
Licensed Physical Therapist in good standing
Experience in post-acute or skilled nursing settings preferred
Strong communication, teamwork, and documentation skills
Proficiency with rehab and EMR systems (Casamba, PointClickCare preferred)
