Position Summary

The Case Manager provides comprehensive case management, care coordination, and recovery support services for individuals receiving outpatient treatment. This role serves as a vital member of the treatment team by coordinating client services, facilitating communication between providers and referral sources, supporting admissions and discharge planning, and helping clients access the resources needed to achieve long-term recovery success. Essential Duties & Responsibilities

Client Care Coordination

  • Coordinate client engagement through appointments, treatment schedules, aftercare plans, meetings, and recovery support services.
  • Maintain regular contact with clients to support treatment engagement and retention.
  • Collaborate with counselors, therapists, medical staff, and other treatment team members to ensure continuity of care.
  • Assist clients and families with navigating treatment, recovery resources, transportation needs, and community supports.

Admissions & Documentation

  • Support the admissions process by ensuring all required consents, documentation, medical records, and referrals are completed and maintained.
  • Scan and upload required records into the appropriate electronic health record system.
  • Conduct client and family orientations in collaboration with the clinical team.
  • Maintain timely, accurate, and compliant documentation of client, family, referral, and provider interactions.

Communication & Provider Collaboration

  • Serve as a liaison between clients, referral sources, healthcare providers, legal agencies, employers, and community partners.
  • Coordinate communication regarding treatment progress, aftercare planning, employment needs, legal requirements, and utilization review activities.
  • Facilitate professional relationships with external providers to support continuity of care.

Aftercare & Recovery Support

  • Develop and coordinate individualized aftercare plans in collaboration with the treatment team.
  • Ensure aftercare services and referrals are established prior to discharge.
  • Provide resource referrals and ongoing support to clients and families.
  • Facilitate groups and activities that complement clinical services and reinforce recovery goals.

Team Participation

  • Participate in multidisciplinary treatment team meetings.
  • Complete all required organizational, licensing, and accreditation trainings.
  • Maintain strict confidentiality and professional standards at all times.
  • Perform other duties as assigned.

Required Qualifications

Candidates must meet one of the following:

  • Has an active statewide clinical license or certification by a statewide certification board.
  • Holds a Master's Degree in a human services field and is actively working toward certification by a statewide certification board (CAADC) or statewide clinical licensure.
  • Holds a Bachelor's Degree in a human services field and is actively working toward certification by a statewide certification board (CADC).
  • Holds an Associate's Degree in a human services field and is actively working toward certification by a statewide certification board (CAAC).
  • Has a High School Diploma or equivalent and is actively working toward certification by a statewide certification board (CAAC).
  • Holds current licensure in the Commonwealth of Pennsylvania as a Registered Nurse (RN), graduated from an accredited school of nursing, and possesses at least one year of qualifying clinical experience. 

Preferred Qualifications

  • Experience working in substance use disorder treatment, behavioral health, mental health, or community-based human services.
  • Experience with case management, care coordination, treatment planning, discharge planning, and resource navigation.
  • Familiarity with electronic health records (EHR) and clinical documentation systems.
  • Strong understanding of recovery-oriented and client-centered care practices.

  • Ability to work independently while maintaining strong collaboration with a multidisciplinary team.
