Job DetailsLevel: ExperiencedJob Location: Gardena, CA 90249Position Type: Full TimeSalary Range: $69.71 - $90.63 HourlyJob Shift: AnyJob Category: Health CareStatement of Purpose    Under the direct clinical supervision of the Medical Director and the administrative supervision of the Health Center Clinic Manager, the Nurse Practitioner is responsible for providing comprehensive primary care services and managing an assigned patient panel. The position works closely with physicians, behavioral health providers, care coordinators, and multidisciplinary care teams to support high-quality, patient-centered care. The Nurse Practitioner provides preventive, acute, and chronic disease management services while supporting population health initiatives, quality improvement activities, and organizational goals.    Major Tasks, Duties and Responsibilities    Provides comprehensive primary care services to patients across the lifespan, including preventive, acute, and chronic care management.   Performs health assessments, physical examinations, diagnoses conditions, and develops appropriate treatment plans.   Orders, interprets, and follows up on diagnostic tests, laboratory studies, and imaging.   Prescribes medications and treatments in accordance with applicable laws, regulations, clinical guidelines, and organizational protocols.   Provides health education and counseling to patients and families regarding disease prevention, wellness, and management of chronic conditions.   Documents all patient encounters accurately and timely in the electronic health record.   Coordinates care with multidisciplinary teams, including behavioral health, care coordinators, specialists, and community resources.   Identifies and addresses care gaps using EHR tools and supports quality improvement initiatives aligned with UDS, HEDIS, and organizational goals.   Participates in population health management, including outreach and follow-up for high-risk patients.   Effectively communicate with team members to address patient needs    Ensure compliance with all federal, state, and local regulations, including HRSA requirements and clinical standards of care.    Participates in peer review, quality assurance, and performance improvement activities.   Supports care delivery in alternative settings as needed (e.g., mobile units or community-based sites).   Maintains patient confidentiality and adheres to HIPAA regulations at all times.   Provides Medications for Addiction Treatment (MAT) services for patients with substance use disorders, including assessment, initiation, and ongoing management in accordance with clinical guidelines and organizational protocols.   Participate in safety programs to promote patient and staff safety.   Maintain professional boundaries and standards of conduct.   Treat patients with respect, care, and concern at all times.   Maintain health and safety standards within the facility.   Recognize personal biases and demonstrate cultural sensitivity when working with diverse populations.   Attend meetings, trainings, and participate in committees as assigned.   Perform other duties as assigned.    Health Center Nurse Practitioner Competencies and Performance Expectations    Ability to complete the above major tasks, duties, and responsibilities.    Ability to demonstrate professional judgment and decision-making.   Ability to adhere to professional standards.   Provides patient-centered, culturally competent, and trauma-informed care.   Communicates effectively and professionally with patients, families, and care team members.   Demonstrates strong organizational and time management skills in a fast-paced clinical environment.   Works collaboratively as part of a multidisciplinary care team.   Maintains accurate, timely, and complete documentation in the EHR.   Adheres to evidence-based clinical guidelines and organizational protocols.   Demonstrates commitment to quality improvement, patient safety, and regulatory compliance.   Maintains professionalism, ethical standards, and confidentiality at all times.    Prerequisite Qualifications    The following are the qualifications and competencies required to perform the essential functions of this position. Qualifications may be subject to modification based on the Americans with Disabilities Act.    Graduate of an accredited Nurse Practitioner program.    Current, active California Nurse Practitioner license.    Current, active California Registered Nurse (RN) license required.    National board certification as a Family Nurse Practitioner or applicable specialty.     Current furnishing number and DEA registration (or ability to obtain upon hire).     Current BLS certification (or ability to obtain within 30 days of hire).     Bilingual (Spanish) preferred but not required.     Experience working in a community health center or with underserved populations preferred, with exposure to integrated behavioral health or co-occurring conditions.   Experience in Medications for Addiction Treatment (MAT) for substance use disorders, including prescribing and ongoing management, or willingness to obtain training upon hire.     Ability to perform physical requirements of the position, including standing, bending, reaching, and lifting as needed to carry out job duties.     Must have valid California driver's license and liability insurance if driving a personal vehicle on BHS business.  Qualifications
