Primary Nurse Coordinator - Adult Community Medical Home

Monday – Friday, standard business hours (with On Call rotation)

KenCrest is seeking a Primary Nurse Coordinator to manage the medical care of the residents within our Adult Community Medical Home (CMH) in Ambler, PA.

At our Adult CMH’s, we provide a home-care environment to four (4) medically fragile adults with intellectual disabilities. Our Care Teams provide 24/7 full circle care to meet all social, medical and emotional needs of our residents while providing them with opportunities for the highest quality life possible.

As our Primary Nurse Coordinator, you will participate in the residents’ care teams, coordinate and schedule all necessary medical appointments and procedures, provide nursing care and supervision to the resident during medical transports, complete all necessary care documentation and monthly reporting, order and maintain DME supply inventory, and provide general administrative support to the program as needed.

Qualifications:

  • Registered Nurse (RN/LPN), licensed in the State of PA     
  • 3 – 5 years' clinical experience, preferred
  • 1 – 2 years’ experience with adult tracheostomy and ventilator management, preferred
  • Valid Driver's License
  • Maintenance of required 30 CEU hrs./2 years
  • Current CPR certification     
  • Clear criminal record and other background checks in accordance with state requirements
