Schedule: Monday-Friday, 8:00am-4:30pm

Location: Dent Tower, Amherst

Responsibilities include:

  • Completes release of information requests including retrieving patient’s medical chart and returning chart, scanning medical record accurately and correctly and transmitting daily, according to requests, established procedures, and established standards of quality and productivity.
  • Date stamps all requests and highlights pertinent data to facilitate processing.
  • Validates requests and authorizations for release of medical information according to established procedures.
  • Performs quality checks on all work to assure accuracy of the release, confidentiality, and proper invoicing.
  • Retrieves medical records for upcoming patient clinic visits, and reviews record for completeness.  Calls referring physician’s office, appropriate clinical laboratories or patient to obtain any missing reports or information necessary for clinic visit.  Ensures completeness of record before patient office visit and reports missing information to appropriate office staff.
  • Sorts, files, and collates a variety of medical record information such as primary physician records or referrals, diagnostic exam reports, laboratory and pathology reports, transcribed clinic notes, operative reports, and discharge summaries into medical records in appropriate sequence and in accordance with established medical record policies and procedures.
  • Adheres to proper naming protocols and is familiar to Eclinical Software.  If paper charts are being accessed will file medical records by alphabet in accordance with established color-coded filing system and predetermined priorities; researches lost or missing records in accordance with established procedures.   Assembles patient medical record as required.
  • Obtains, when required, medical record releases from patients in order to retrieve and copy medical information from patient medical records to answer authorized requests of patient care activities, patient account settlements, and third party sponsorship, documenting patient records appropriately.
  • Answers telephone and takes verbal and e-mailed requests for medical records or information; sorts requests according to predetermined priorities and provides routine information of a general nature such as demographics, clinic dates, or record locations to authorized requestors.
  • Support Central Delivery with helping to distribute incoming mail to various parts of the organization.


Compensation:

  • Most candidates will start within the first quartile of the pay range
  • Rates are commensurate with experience
