Quality Assurance Coordinator

FLSA: Non-exempt

Reports To: Director of Operations

Physical requirements: General good health; must sit at computer for extended periods of time.

Hours of Work: Full time 37.5 hours per week. Weekend or evening work required as requested.

 

Job Summary: Functions as a resource and support to all agency programs and individual's we support, including CCL Delaware, in the provision of Council on Quality & Leadership (CQL) personal outcomes. Oversees quality initiatives connected to CQL, OPWDD, and self-advocacy. Advocate for and mentors' people with disabilities.

 

Job Qualifications:

  1. High school diploma/GED required. Bachelor's degree preferred.
  2. Three years' experience working with people with disabilities required.
  3. CQL and/or POMS certification and experience preferred. Experience and working knowledge of NYS regulation with OPWDD, DOH, OMH and/or quality assurance activities preferred.
  4. Good communication and interpersonal skills are required.
  5. Valid NYS driver's license that meets agency standards and reliable transportation.
  6. Must be able to read and write the English language.

 

Major Responsibilities:

  1. Attain and maintain CQL certification as POMs interviewer and trainer for the required certification period.
  2. Coordinate trainings for selected staff to become CQL certified POMs interviewers.
  3. Schedule and ensure completion of Personal Outcome Interviews for all persons supported in Day, Residential and FSS programs; ensure interviews are thoroughly completed on a timely basis.
  4. Assist the Agency with obtaining CQL accreditation through development of a comprehensive project plan; track performance and ensure adherence to established timeframes.
  5. Develop and maintain tracking/trending system for all elements of CQL, and produce quarterly reports on this information; review data and make recommendations for quality improvement. Enter data in CQL Portal as prescribed.
  6. Coordinate and deliver on-going CQL/POMs training and education to staff and individuals we support. Conduct quality initiative training for New Hire Class and as assigned.
  7. Maintains a working knowledge of regulations and agency quality assurance standards.
  8. Chair Dignity in Life Committee and serve on agency committees as assigned.
  9. Develop and conduct satisfaction surveys for people supported annually and flash surveys for family members monthly.
  10. Advise Self-Advocacy Group and partner with Capital Region SANYS.
  11. Mentor people receiving services who are on committees.
  12. Participate in CQL Community of Practice.
  13. Assist Director of Compliance and Quality Assurance with other quality improvement activities as assigned.
