Homelessness is the largest social and public health crisis in California. Illumination Health & Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IH+H currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.


Role:

The Manager of Medical is responsible for overseeing and optimizing the discharge care plan of homeless patients from hospitals or other agencies contracted with IHH’s Recuperative Care Centers, while providing high-quality medical follow-up coordination and social care while overseeing community relations and operational standards.

The salary range for this position is: $75,000 - $90,000 per year
The schedule for the role is: Monday - Friday, 1pm - 9:30pm


Responsibilities:

Client Care
  • Provide optimal client centric care to overcome any Social Determinants of Health
  • Verify all tasks requested by Funders are completed.
  • Have the capacity to provide and train others in a culturally and linguistically competent manner.
  • Monitor high acuity client for ongoing medical or behavioral health complications.
    • Review and supervise complex cases and act as the lead contact for clients or provider inquiries/concerns in obtaining appropriate care for the clients.
  • Oversee and manage the successful Medical Oversight and Monitoring of the Medical Coordination Department for assigned locations/ regions to the standard.
    • Medical Oversight and Monitoring include but limited to the following:
    • Medication Review and Education
    • Health care education
    • Self-care instructions and education
    • Making appointment with healthcare providers
    • Accessing treatment and health care at appropriate points of care
    • Linkage to services
  • Verify initial data collection to ensure clients discharge from hospital or referring agency was appropriate and all needs for the continuation of care can be addressed successfully.
  • Maintain ongoing tracking and appropriate documentation on client services to promote team awareness.
Community Relations
  • Identify and utilize cultural and community resources.
  • Establish and maintain relationships with identified service providers.
  • Answer inquiries from clinical teams and patients, provide information, and resolve complaints from the public or other agencies in regard to Medical Oversight and Monitoring.
  • Utilize effective written and verbal communication skills to provide appropriate responses in a timely fashion.
Supervision and Management
  • In coordination with the Site Manager, will plan, organize, direct, coordinate and supervise the operations of the Medical Coordination departments.
  • Promoting confidence in employees by providing structure and organization within the company.
  • Provide oversight of daily operations for the department including staffing, training, metrics, performance, and reporting.
  • Supervise, direct, train and evaluate performance of all assigned staff.
    • Supervision of staff includes but limited to:
      • Performance Review’s.
      • Review and Approval of Timecards
      • Correctional Action for Misconduct.
      • Ensure Staff is preforming to standards and maintaining professional demeanor.
  • Interview, suggest best candidates to hire, perform disciplinary action and performance evaluations for employees.
Administrative/Record Keeping
  • Enter client information and case notes into KIPU and other documentation systems.
  • Complete and update logs.
  • Perform administrative tasks such as answering phones and filing paperwork.
  • Prepare client reports for billing.
  • Maintain proper inventory of supplies.
Compliance/Safety
  • Maintain confidentiality in accordance with HIPAA guidelines and IF standards.
  • Provide clients with proper compliance forms upon admission to maintain client compliance with program rules.
  • Provide care utilizing infection control measures that protect both the staff and the client according to OSHA standards.
  • Maintain a safe work environment.

  • Communicate any concerns and feedback to Supervisor with tact and professionalism.
  • Set and maintain healthy boundaries with clients.
  • Possess knowledge of medical terminology, diagnosis, and medications.
  • Demonstrate the ability to maintain confidentiality of client information and work-related information/materials

Preferred Experience/Minimum Qualifications:

Required:
  • High School Diploma or equivalent.
  • Current and non-restricted LVN or RN license in California
  • 2 years of relevant experience.
  • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
  • Valid CA Driver’s License.
Preferred:
  • Home Health nursing experience and experience working with populations dealing with addiction and mental health conditions
  • Bilingual in English and Spanish.
  • Proficiency in Google Suite (Mail, Docs, Sheets, Calendar) and KIPU or other electronic health record systems.
