A Case Facilitator I - Child and Adolescent operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to child and adolescent consumers.
o   Links, monitors, and refers clients to appropriate community resources
o   Coordinates care with appropriate internal and external parties
o   Ensures appropriate behavioral health services are provided
Education: High school diploma or GED, equivalent.
Experience: Must meet OBHP requirements = Two years’ experience in mental health or community services preferred.
Licenses/Certification: Valid state issued drivers license.
This is a full-time community based position M-F with a semi-flexible schedule, with a preference of 1-2 evenings until 7pm.Â
