Matthew 25’s Mission:

Key Responsibilities
- Conduct thorough patient assessments, including medical history reviews, physical examinations, and symptom evaluations.
- Developing and implementing patient-centered treatment plans, which may involve prescribing medications, therapies, and other interventions.
- Educating patients about the prescribed treatments and expected outcomes.
- Monitoring patient progress through regular follow-ups and adjusting treatment plans as necessary.
- Conducting screenings, immunizations, and health assessments to prevent illnesses and promote wellness.
- Maintaining accurate and detailed medical records, including patient histories, progress notes, and treatment plans.
- Engaging in ongoing professional development activities to stay current with advancements in healthcare practices
- Participating in quality improvement initiatives and promoting evidence-based practice.
QUALIFICATIONS
- MSN in Nursing required.
- Maintain a current and valid M.D., D.O., P.A. or N.P. Kentucky and/or Indiana license, along with national certification.
- 2 – 3 years family practice or primary care.
- Experience working in a nonprofit organization or a strong desire to work in service driven environment.Â

- Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
- Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
- Must be able to pass a drug screen and background test.
