Position Details:
Utilizes the nursing process to provide patient care under the supervision of person(s) licensed by a State regulatory board to prescribe medication and treatment, or under the direction of a Registered Professional Nurse (RN). Maintains professional accountability for provision of patient care for the assigned patients. Coordinates the patient care in conjunction with other departments.
•   Involves patient and/or significant others in decisions regarding care
•   Assists the RN in admission and discharge process
•   Performs patient assessments and rounding as indicated by the patient's condition or as directed by the RN
•   Performs patient handoffs utilizing patient safety tools both from shift-to-shift and interdepartmentally
•   Facilitates communication of pertinent findings including significant changes in patient condition to RN in a timely manner
•   Maintains effective communication through appropriate utilization of the chain of command
•   Communicates subjective and objective data about the patient/family and/or significant others with appropriate members of the multidisciplinary team
•   Collaborating with the RN, evaluates patient response to care and in determining progress toward achievement of outcomes
•   Documents nursing interventions toward the patient/family promoting optimal health, healing, or dignified end of life when appropriate
•   Reinforces and appropriately documents the educational needs and learning abilities of the patient and/or significant others during the admission and subsequent assessments in the computerized care plan
•   Collaborating with the RN, prioritizes care delivery according to the patients needs and unit activities
•   Delegates, guides, and directs UAPs in tasks and is responsible for the care provided according to their documented competencies
•   Completes hospital and required nursing competencies, mandatory in-services/assignments and relevant certifications within required time frame
•   Stays up-to-date on unit huddles, email, and other unit-based communications
•   Participates in Process Improvement activities such as data collection, in-services, and meetings as appropriate and required
•   Additional duties as assigned
Minimum Qualifications:
Education: Graduate of an accredited practical nursing program; Consideration will be given for comparable education experience and successful licensure.
Licensure: Is personally responsible for obtaining, and maintaining a current LPN license within the hiring state in which nursing duties are performed and must meet all State Board of Nursing requirements
Experience: Can be new or experienced LPN
Certifications: Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire
NOTE: one or more of the certifications below may be required based on the position/unit hired to, or acquisition of certification within department required timeframe:
•    ACLS (Advanced Cardiac Life Support)
•    PALS (Pediatric Advanced Life Support)
•    PEARS (Pediatric Assessment Recognition and Stabilization)
•    S.T.A.B.L.E. (Sugar, Temperature, Airway, Blood Pressure, Lab Work, And Emotional Support)Â
•    Six Assessment & Care Modules
•    Other unit-specific certifications as required
Physical Requirements/Demands: Stand/Walk most of the time on duty; significant amount of bending and stooping, as well as lifting and turning patients with assistance; push/pull/grip up to 50 pounds
Preferred Certifications: IV Certification within 1 year of hire
