The Lutheran Home Skilled Nursing Community is hiring a Social Services Coordinator to support resident well-being and coordinate psychosocial services for our residents and their families.
What You'll Do
- Complete social service assessments to identify each resident's needs and preferences
- Coordinate discharge planning and connect residents with outside psychiatric and psychosocial services
- Advocate for residents and support families through care transitions
- Facilitate family involvement in care planning meetings
- Connect residents with community resources and encourage participation in facility programming
- Document social service plans and progress notes accurately
- Collaborate with nursing, therapy, and the rest of the care team on resident care plans
What You'll Need
Required:
- Minimum 2 years of experience in a Social Services role within a skilled nursing facility
- Direct experience with discharge planning in a SNF setting
Preferred:
- Bachelor's degree in social work, human services, or a related field
- Experience working with Medicare/Medicaid resident populations
