What You Will Do:
路聽聽聽聽聽聽 Obtain/verify physician order for therapy and validate for appropriateness.
路聽聽聽聽聽聽 Review available medical records and obtain pertinent medical history from therapist documentation and patient report.
路聽聽聽聽聽聽 Ensure that patient has given informed consent to treatment.
路聽聽聽聽聽聽 Identify problems that need further assessment by therapist and possible change to treatment plan.
路聽聽聽聽聽聽 Identify patient accomplishment of goals and report to therapist that goals need to be updated or revised.
路聽聽聽聽聽聽 Follow plan for care, or appropriate therapeutic intervention to achieve stated goals.
路聽聽聽聽聽聽 Participate in discharge planning for patient.
路聽聽聽聽聽聽 Demonstrates competence in the performance of individualized care based on age specific needs.

路聽聽聽聽聽聽 Accurately submits patient charges and records procedures according to departmental policy.
路聽聽聽聽聽聽 Documents assessments of education needs, education provided, and patient understanding of education provided in accordance with departmental, TJC, Medicare, and other applicable regulatory guidelines.
路聽聽聽聽聽聽 Completes discharge summary for each patient in accordance with departmental, TJC, Medicare, and other applicable regulatory guidelines.
路聽聽聽聽聽聽 All documentation is completed within established time frame.
路聽聽聽聽聽聽 Accurate timely documentation and reporting of unusual occurrences, on the job injuries, and medical device malfunctioning.
路聽聽聽聽聽聽 Completes required documentation tools for outpatients and for specialty units/programs (i.e. Lysholme, MDS, staffing reports, etc.)
