Child & Adolescent Grief Coordinator

Summary

Provide high quality, professional grief services care primarily to children, teenagers and their families admitted to the Grief Services Program from hospice care, palliative care and the community.

 

Essential Duties & Responsibilities

  •  Provide grief support through in-person counseling, telephone support, and grief programs to children and teenagers admitted to our Compass program as well as their family members, as appropriate, through a safe, therapeutic, and healing environment
  • Assess and develop Individualized Care Plans to address Compass clients identified psychosocial needs and goals as related to their grief
  • Create concise, objective focused clinical notes, documenting information pertinent to the plan of care through assessment, plan, intervention, and evaluation of Compass services provided to               clients   
  • Collaborate with team members when needed 
  • Coordinate events, school-based grief support groups, and other programs that provide support to the different developmental and psychosocial needs of children and adolescents.
  • Introduce and facilitate rehearsal of techniques to aid immediate and long-term coping, with consideration for the unique needs of the individual and family, such as coping style, previous experience, developmental level, culture, spirituality, family situation and emotional state.
  • Adhere to the practice of confidentiality regarding client and family information
  • Discharge planning for clients being discharged from Compass Bereavement Care
  • Provide grief counseling to children and teenage members of hospice and palliative care patients when appropriate and in accordance with guidelines

  • Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
  • Knowledge of HIPAA privacy rules and regulations
  • Knowledge of electronic medical records systems

Minimum Qualifications

Education: 

  • Bachelor’s degree in Social Work, counseling or related fields - Master's degree preferred   

Experience: 

  • 2 years’ work experience in client-care setting with children and adolescents individually and/or in groups    
  • Counseling, specifically in the areas of grief/loss preferred   

Licensures & Certifications: 

  • Licensure/Certification for designated educational background 
  • Valid driver’s license and automobile insurance 

 

Summary of Physical Requirements

  • Hand dexterity to include gripping, carrying, pushing & pulling 
  • Fine finger movements to include writing & typing, frequently 
  • Flexion/Extension of arms & wrists 
  • Additional gross motor movements including: bending, reaching, twisting, turning, crouching, squatting, & kneeling   
  • Near & far visual acuity  
  • Ability to hear, understand, & distinguish speech and other sounds  
  • Ability to drive and/or transport self to and from work locations 
  • Ascending or descending stairs & ramps 
  • Ability to lift, carry, push & pull up to 10 pounds, occasionally
