About Us
Ashland City Clinic is a patient-centered Family Medicine practice located in Ashland City, Tennessee. We are expanding our integrated behavioral health services to better serve our community, which has significant unmet mental health needs. By embedding licensed counselors directly within our primary care setting, we provide patients with seamless access to mental health care — reducing stigma, improving follow-through, and delivering whole-person care under one roof.
The Opportunity
We are seeking an experienced Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) to join our practice as a 1099 independent contractor. This role combines in-person counseling at our Ashland City clinic with telehealth services for patients across our service area. - Conduct comprehensive biopsychosocial assessments, develop treatment plans, and provide evidence-based interventions
- Deliver services both in-person at the Ashland City clinic and via telehealth to patients in the broader service area
- Collaborate with primary care providers on integrated behavioral health care, including warm handoffs, care coordination, and shared treatment planning
- Provide brief interventions and crisis stabilization as needed within the primary care setting
- Maintain timely and thorough clinical documentation in the electronic health record (eClinicalWorks)
- Participate in care team meetings and contribute to quality improvement initiatives
- Coordinate referrals to community resources, psychiatric services, and higher levels of care when appropriate
- Maintain compliance with all Tennessee licensing requirements, ethical standards, and HIPAA regulations

- Experience with evidence-based therapeutic modalities (e.g., CBT, DBT, motivational interviewing, solution-focused therapy)
- Comfortable providing both in-person and telehealth services
- Strong clinical judgment, professional boundaries, and ethical practice
- Excellent communication and collaboration skills for working within an integrated care team
